Includes approximately eight scheduled service visits, up to eight TLES/workforce/life-skills sessions, client engagement and navigation, field documentation, referral tracking and monthly reporting.

Includes a customized combination of scheduled onsite support, community engagement, TLES programming, workforce preparation, resource navigation, client escorts, documentation, program coordination and detailed monthly reporting.
